About Breast Augmentation
Breast augmentation is a surgical procedure that uses breast implants to improve the appearance of the breasts. Implants can increase the breasts size, enhance their shape, restore volume and correct asymmetries. Implants can also reconstruct breasts after cancer treatment or injury.
Breast augmentation is also known as augmentation mammaplasty and the procedure is consistently the most popular cosmetic plastic surgery performed in the United States.
Ideal Candidates for Breast Augmentation
The best candidates for breast augmentation are healthy and non-smoking. These patients want an enhanced breast appearance, and have realistic expectations of what the procedure entails and what will be achieved.
Some patients may need a breast lift along with breast implants, and others may be better suited for a breast lift alone.
Patients who are not good candidates for breast augmentation are those currently pregnant, breastfeeding, or with active breast disease.
Breast Augmentation Consultation
The first step of a breast argumentation is a consultation with Dr. Mahmood at her Union Square, New York practice.
The breast augmentation consultation is an opportunity for Dr. Mahmood to understand each patient’s unique motivations, expectations, the desired look for their surgery, as well as gather medical information.
The consultation begins with a conversation about patient goals and Dr. Mahmood then performs a thorough review of the patient’s medical history, medications, smoking status, and previous surgeries. She next completes a physical exam to evaluate chest measurements and any asymmetries. All of this information is used to tailor a treatment plan and help patients determine the right implant size.

The consultation is also an opportunity for patients to learn about the types, sizes, locations and positions breast implants. These elements present customizable choices that must be finalized prior to surgery.
Types of Breast Implants
Implants can be silicone or saline filled. Both types are FDA-approved for use in cosmetic and reconstructive breast surgery.
Saline implants have a smooth, silicone shell, and are filled with saline solution. Regulations state that these are the only implant that can be used in patients under 22 years old. While the benefit of these implants is that rupture is detected immediately because the implant will quickly become flat and the saline will be absorbed by the body, the downside is that they look and feel less natural and can cause a rippling effect under the skin which is quite unsighly to patients.
Silicone implants also have a smooth, silicone shell but are filled with a cohesive, or ‘gummy bear’ type gel consistency. While detection of rupture requires an MRI, the newest generation of silicone implants has an improved safety profile because of the cohesive gel. The main benefit of silicone implants is that they look and feel more like natural breast tissue, and they are the most common type of implant that is used.
Breast Implant Size
Breast implants are available in a wide variety of sizes and projections. Dr. Mahmood believes it is important for a patient to personally select and be excited about the implant size, and she will guide you during this process. Based on the measurements of your chest and what your body frame can accommodate, she will review the variety of sizes and projections that are possible. Her office has actual samples of the range of sizes and projections, so patients can actually try on the implants to see what different options would look like.
Surgical incisions can be made in the armpit, areola, or under the breasts in the fold. Dr. Mahmood prefers under the breast in the fold. This area has the least amount of bacteria and therefore is thought to have the lowest risk of introducing bacteria to the implant, decreasing the risk of capsular contracture (when the body forms a thick capsule around the implant inside). In addition, various patients tend to form keloid, or thickened scars. An incision under the fold tends to be well hidden and the weight of the breast helps the scar become smoother and flatter.
Breast Implant Position
After the incision is made, the breast implant is placed in a pocket in the chest. The implant can be placed under the muscle (subpectoral), under the covering of the muscle (subfascial), or over the muscle (subglandular). Most commonly, the implants are placed under the muscle or over the muscle. Both approaches are safe and reasonable options, and Dr. Mahmood will recommend the preferred approach based on your body characteristics and chosen implant.
The benefits of subglandular implant placement are that there may be less initial post-operative discomfort. The disadvantages
are that many women do not have adequate breast tissue to cover the implant and rippling and the implant edge may be visible. Also, the risk of capsular contracture, or hardening of the breast due to the body forming excessive scar tissue capsule around the implant, is increased when the implant is placed above the muscle.
Submuscular, or under the muscle, placement provides a more natural looking breast appearance, provides more support for the implant and reduces the risk of capsular contracture. This approach is preferred in most women desiring breast augmentation since there is generally thin breast tissue and skin and the muscle adds another layer covering the implant.
Some patients desiring breast augmentation may have loose, sagging breast tissue that also requires a breast lift. Augmentation mastopexy is the term used for a breast lift with implants. During your consultation, Dr. Mahmood will discuss your desired result and aesthetic goals, and perform a thorough physical exam to tailor the appropriate surgical treatment for you – whether it is a breast lift alone, breast augmentation alone, or breast lift with implants. Augmentation mastopexy is a more complex surgery as it involves various elements including adding volume to the breast, and also tightening and reshaping the overlying breast tissue. Breast Augmentation Surgery
Breast augmentation surgery begins with marking the procedure and the breast landmarks.
Next, patients undergo general anesthesia by a Board-Certified Anesthesiologist.
Once anesthesia is administered, the augmentation procedure is performed according to the decisions made during the consultation about incision, implant type and size, and pocket placement.
The breast pocket is irrigated with antibiotic solution, and the implant is inserted. Next, the breast pocket is closed in a layered manner to provide added security to the implant, and to make sure the outside scar heals as a small, fine line.
Throughout the procedure Dr. Mahmood meticulously ensures that the implants are symmetrically placed, and adjusts as necessary to accommodate for any asymmetries.
The sutures used in surgery are all dissolvable, and the surgery itself takes less than one hour. At the end of the procedure Dr. Mahmood will place gauze on the incisions for comfort and the soft supportive compression bra.
Recovery & After Care
After surgery, Dr. Mahmood sees patients at her Union Square office at one day, one week, one month, and three months post-op. Additional visits are recommended as needed. At post-operative visits, Dr. Mahmood guides patients on when to increase physical activity, optimal scar treatment, and steps to take to ensure a beautiful final result.
In general, patients are advised to wear a soft supportive surgical or sports bra for one to two months. Vigorous exercise or heavy lifting should be avoided for one month after surgery.
Breast augmentation results are noticed immediately after surgery, but the final outcome is appreciated after approximately three months after surgery when the implants have settled, the breast tissue has softened, and the swelling has resolved.
